Where do I park when coming to your office?
Free parking is available in the back of our building on a limited basis. Additional free street parking is available nearby. Please be sure to check posted street signs for any restrictions.
What payment methods do you accept?
We accept cash, credit cards, Venmo, and HSA/FSA payments. Payment is due at the time of service unless prior arrangements have been made.
Can I reschedule or cancel my appointment?
Yes. At Koah Health, we understand that unforeseen circumstances may require you to cancel or reschedule your appointment. To accommodate all patients effectively, we have established the following cancellation policy:
Notice Requirement: Please inform us of any cancellations or rescheduling at least 12 hours before your scheduled appointment time.
Late Cancellation Fee: Cancellations made with less than 12 hours’ notice may incur a fee equivalent to $50, as this time frame often prevents us from offering the slot to another patient in need.
No-Show Policy: If you do not attend your scheduled appointment without prior notice, a no-show fee of $50 will be applied.

What physical therapy services do you offer?
We provide comprehensive physical therapy, treating both acute and chronic conditions and post-surgical rehab. Manual therapy is at the heart of what we do, utilizing myofascial release, visceral mobilization, craniosacral techniques, in addition to therapeutic exercises, and posture correction. These services are tailored to support recovery, manage chronic conditions, and improve overall health.
Do you take insurance?
Yes, we are in-network with many major insurance plans. Because coverage varies by plan, our team is happy to help verify your benefits before your first visit. You can refer them to our Insurance Verification page for a list of our accepted insurance providers.
What if my insurance doesn’t cover your services?
If your insurance doesn’t cover our services, we offer self-pay options and can provide you with detailed receipts to submit for out-of-network reimbursement.

What conditions do you treat?
We treat a wide range of conditions across the lifespan using an integrative physical therapy approach. This includes:
- Orthopedic and musculoskeletal pain or injuries (neck, back, shoulder, hip, knee, TMJ)
- Pelvic floor conditions, including pregnancy- and postpartum-related concerns
- Infant conditions such as torticollis, positional asymmetries, and movement development support
- Neurological and nervous system–related symptoms (headaches, dizziness, post-concussion symptoms)

How should I prepare for my first visit?
Preparing for your first appointment at Koah Health ensures a smooth and productive experience. Here’s what you need to know:

Your first session lasts about 60 minutes and includes a review of your medical history, a physical assessment, and a discussion of your goals. We’ll create a personalized treatment plan to meet your needs.
Do I need a prescription from my doctor to come see you?
In California, patients have direct access to physical therapy services without a physician’s prescription. This means you can begin treatment at Koah Health without prior referral. However, state law requires that after 45 calendar days or 12 visits—whichever comes first—a physician must sign off on your treatment plan to continue therapy. Additionally, some insurance providers may require a physician’s referral for coverage.
Functional Medicine
What functional medicine services do you offer?
We provide personalized care, including comprehensive health assessments, advanced lab testing, and tailored treatment plans. Services focus on nutrition, lifestyle changes, stress management, and supplementation to address root causes and improve overall health.
How are Functional Medicine services offered?
Functional Medicine services are offered as a program that begins with an initial evaluation and includes three follow-up sessions. During the initial evaluation, general recommendations and next steps will be discussed. If you choose to opt into the program, the remaining three sessions will be scheduled every two weeks or once per month based on your needs. After completing the program, individual sessions may be purchased for continued care.
This approach allows us to establish a strong foundation by addressing immediate concerns during the initial evaluation while providing ongoing support to implement and refine personalized recommendations. The structure ensures consistent monitoring and adjustments, empowering you to achieve sustainable health outcomes.

Do you take insurance?
Our Functional Medicine services are not covered by insurance. While we do not accept insurance directly, we will work with your other providers whenever possible to get services like basic lab testing covered by your insurance plan. We do accept HSA and FSA payments for Functional Medicine services.

The Functional Medicine program addresses chronic conditions such as digestive disorders, hormonal imbalances, autoimmune diseases, chronic fatigue, weight management challenges, and metabolic dysfunction. It is also beneficial for those seeking support with stress-related health issues, inflammation, or optimizing overall wellness.
How should I prepare for my first Functional Medicine visit?
A link to the intake forms will be emailed to you. It is important to thoroughly complete the forms with accurate and detailed information to make the most of our time together. Include information about any recent lab work, relevant medical records, and a list of current medications and supplements. We will go over this information together as we create a detailed health history and formulate a customized treatment plan. Please note, sessions will be rescheduled if forms are not completed 24 hours in advance.

WELLNESS SESSION
What are wellness sessions?
Wellness sessions are non-insurance, self-pay sessions focused on maintaining progress, supporting overall movement quality, and promoting long-term physical well-being. They are designed for individuals who want continued support beyond traditional physical therapy or who are not seeking insurance-based care.
How are wellness sessions different from physical therapy?
Physical therapy is a medical service billed to insurance and requires a diagnosis and plan of care. Wellness sessions are not billed to insurance and do not require a medical diagnosis. They focus on movement, education, and hands-on care to support function, comfort, and resilience.
Who are wellness sessions a good fit for?
Wellness sessions may be appropriate if you:
- Have completed physical therapy and want ongoing support
- Are managing chronic tension or stress-related movement patterns
- Want guidance with exercise, mobility, or body awareness
- Prefer a non-insurance, maintenance-based approach to care
What happens during a wellness session?
Each session is individualized and may include movement coaching, manual therapy, breathing strategies, and education based on your goals and needs. Sessions are designed to support how your body functions as a whole.
Are wellness sessions covered by insurance?
No. Wellness sessions are self-pay and are not billed to insurance. Payment is due at the time of service.
Do I need a referral for wellness sessions?
No referral is required for wellness sessions.
If you don’t see your specific concern listed, we encourage you to reach out. Many of our patients present with overlapping or hard-to-define symptoms, and we take a whole-person approach to care.
